In practice, it seems that everyone is collecting the same data over and over again. Medical practices have you fill out forms over again and over again. The forms and questions always seem a little bit different, but does the data get better with repetition.

The other funny thing about data is that everybody thinks that their data is correct until given documented proof otherwise. The amount of data that you need to override someone else’s bad data can be quit significant. It
